Will be hospitalized on election day (or a member of his/her family will be hospitalized on election day). 4. Has a personal illness or physical disability.

How Can I Acquire an Absentee Ballot?
Any person wishing to vote by absentee ballot should write to or visit the board of elections in his/her county and request an absentee ballot, giving the
following information (a form is available from the county board of elections for convenience, but is not required):
- Voter's name and voting residence.
- The election for which the voter is requesting a ballot. - The reason for being absent from the polls on election day (see absentee qualifications). - Voter's party choice if the election is a
primary or a note stating that the voter wants to vote an issues-only ballot. - The mailing address to which the voter wants the ballot sent. - Voter's signature.
Voters should request an
absentee ballot as early as possible in order to meet the return deadline. When making a written request for an absentee ballot through the mail, the application must be received by noon three days
before the election.
Those wishing to vote absentee in person may apply and vote up until closing time at the Board of Elections Office the day before the election.
How do I Return the Absentee Ballot and What is the Deadline?
All voted absentee ballots must be received at the board office by 7:30 p.m. on election day in order to be counted.
Other Information on Absentee Voting in Ohio
The absentee ballot request deadline is 3 p.m. on election day in the case of medical emergencies. To be eligible under this provision, a voter must be confined
in a hospital because of a medical emergency. A close relative may deliver and return the ballot so request (though the relative's name must be stated on the ballot), or representatives of the board of
elections can be requested to deliver the ballot.
